CANCELLATION POLICY
We understand that unexpected events or circumstance can come up that could prevent you from keeping your appointment. If you are unable to keep an appointment, we cordially ask that you provide us with more than 48 hours notice. As a courtesy to doctors and other patients, We require this advance notice so that we can offer this appointment to another patient that may be in need of our services. In addition, our growing practice still needs to cover our overhead expenses for the day. A fee will be charged if a patient does not show up for an appointment or does not provide more than 48 hours or 2 (two) business days of notice. We would like to take this opportunity to thank you for understanding and for choosing our practice for your dental care.

We will be more than happy to submit all insurance forms for you. Please remember you are fully responsible for all fees charged by our office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.
